This transition pack using resources from Dot Com Digital has been created to support teachers and pupils primarily in Year 6 (the content could also be used with Year 7). The content covered in the pack could be used by teachers to form the basis of a Junior citizenship or Crucial Crew type series of sessions; perfect to use this term to support transition.
